Supported Living
HOPE provide high-quality supported living solutions for adults who need a flexible and supportive living arrangement.
Our Supported Living service allows people living complex care needs to live as independently as possible in their own homes, whilst giving them the encouragement and reassurance that extra support is available.
We are experienced in delivering personalised care and supported living services to young people in transition to adulthood, helping them on their journey to independence.
We enable and empower young people to make a successful transition from care, to living independently in the community. We work in close partnership with the people we support, their families, carers and health professionals, to ensure we are offering the right services to suit the individual needs of each young person.
We deliver high quality support services in association with Care Quality Commission (CQC) and we are able to offer support to:
- Homeless families and individuals
- People with physical disabilities
- People with learning disabilities
- People with mental health needs
Our Supported Living Service provides modern, accessible facilities that are maintained to the highest standards.
