Reablement
Reablement, also called intermediate care, is available to accommodate people who need help after being discharged from hospital, or recovering from an illness or injury.
Our reablement service is available to older adults, people with disabilities, or anyone requiring assistance with daily living activities.
Once they are in our care, on our daily visits we try to get them back into their routine by assisting ,directing and empowering the individual to do things for themselves and become independent again.
We will monitor their progress and set them objectives reviewing it on a daily basis until they are able to do things unaided activities, eg personal care, cooking etc.
