Reablement

Reablement

Reablement, also called intermediate care, is available to accommodate people who need help after being discharged from hospital, or recovering from an illness or injury.

Our reablement service is available to older adults, people with disabilities, or anyone requiring assistance with daily living activities.

Once they are in our care, on our daily visits we try to get them back into their routine by assisting ,directing and empowering the individual to do things for themselves and become independent again.

We will monitor their progress and set them objectives reviewing it on a daily basis until they are able to do things unaided activities, eg personal care, cooking etc.

Care and support to help you get back on your feet!

Why use HOPE reablement care?

Reablement care can be a lifeline for people recovering from illness, surgery, or a decline in mobility. It provides temporary but crucial support to help individuals regain their independence and confidence in daily living. This could involve tasks like getting dressed, preparing meals, or managing medication.

By offering personalised support services such as therapy, home help, and social interaction, reablement care can significantly improve recovery rates and overall well-being.

It empowers people to return to their own homes sooner, reducing the risk of hospital readmission and promoting a sense of dignity and control over their lives.
