End of Life
HOPE recognises and fully supports a service user’s End of Life Care Pathway, which is a holistic, ‘whole-person’ approach to end of life care and dying. We focus on making sure that the final weeks and days of a person’s life are comfortable, so that they can pass in the most peaceful way possible.
Our EOL care at home service is available for those who wish to spend their last moments within their own home surrounded by their beloved family members.
HOPE respects “One Chance to Get it Right’. Therefore, a holistic approach to recognising an individual’s culture, religion, spiritualism, gender etc remains paramount. Equally, continuity and consistency of care, as it is distressing for the user and family members if workers are changed.
OUR AIM
- To recognise and fully support a Service Users ‘End of Life Care Pathway’ and offer care when you need it most.
- To provide holistic support needed at the end of someone’s life.
Caring for your loved one and giving them peacefulness and comfort as they approach their final days.
We ask all Service Users if they want to record their wishes regarding EOL and if they want an EOL Plan. We also respect and document Advance Care Plans too, which are referenced within their personalised Care Plan.
All communications are sensitive with the person who is dying and those important to them, likewise we listen and respect. Therefore, HOPE will work closely with palliative care practitioners to help implement the care plan that has been agreed. Likewise with District and MacMillian Nurses, palliative care practitioners and GPs etc.
We are able to provide support on a 24-hour basis, if required.
End-of-life care at home allows you – or a loved one – to remain in the place you feel most comfortable and safe as you approach the end of your life.
After a terminal diagnosis, change can be disruptive and unbalancing, as can the process of settling into a completely new environment such as a hospice or hospital.
Fully-managed end-of-life home care lets you or a family member stay at home, surrounded by loved ones and special comforts, while receiving care that focuses on managing symptoms and minimising pain levels.
How can End of Life care help you?
At HOPE, we focus on promoting independence and respecting your dignity and privacy. Our fully trained carers offer regular support with a wide range of tasks to make daily life a bit easier for you or a loved one.
Your local care manager will work with you to understand what your specific needs are and the different ways your chosen carer can support you, such as:
Personal care
- Supporting with bathing and showering
- Toileting and changing continence pads or using a commode
Mobility support
- Help moving position in bed
- Help with standing or sitting
- Help using hoists when required
Administering medication
- Administering or prompting medication
- Collecting prescriptions and supporting with pain management
Housekeeping assistance
- Undertaking duties including cleaning and vacuuming
- Changing bedding
Complex care
- Support with catheters
- Stomas and PEG feeding
- Other conditions and specific support
Food & Nutrition
- Preparing meals and drinks
- Serving meals at the times of day that you choose
